Real Time Medical Systems to Present Live Analytics Strategies to Advance Value-Based Care Outcomes at PALTC26

Company collaborates with leading I-SNP Provider Partners to share insights that support chronic disease management, reduce rehospitalizations, and improve quality of life in post-acute care

BALTIMORE--(BUSINESS WIRE)--Real Time Medical Systems (Real Time), the industry-leading, KLAS Rated, and HITRUST Certified post-acute analytics solution, announced today its participation in PALTC26, the annual conference of the Post-Acute and Long-Term Care Medical Association (PALTmed), taking place March 26–28 in Anaheim, CA. As providers across the post-acute care continuum face growing demands of value-based care environments, Real Time and Provider Partners will highlight how timely, actionable analytics help care teams translate strategy into daily practice – supporting more informed clinical decisions, stronger interdisciplinary coordination, and improved patient outcomes.

“In value-based care, outcomes are shaped by how well teams anticipate & respond to change. Access to live clinical insights allows the care teams we serve to prioritize more effectively, align across disciplines, & stay focused on patients most at risk."

As part of the PALTC26 educational agenda, Phyllis Wojtusik, Executive Vice President, Value-Based Care of Real Time, will present alongside Dr. Alan Kimmel, Chief Medical Officer of Provider Partners, “Strategies to Enhance Value-Based Outcomes Through Live Analytics” on Thursday, March 26, 2026, from 4:30pm to 5:30pm PT.

The session will focus on how nursing facility teams are leveraging live insights to detect changes in condition earlier, manage chronic disease more effectively, and intervene before emerging clinical risk leads to avoidable hospital utilization. Attendees will learn how analytics‑enabled workflows support operational efficiency and performance measures such as Case Mix Index (CMI), while advancing patient-centered care.

“Nursing facility care teams play a vital role in value-based care success. Achieving strong value‑based outcomes depends on how quickly organizations can recognize clinical signals as they emerge,” said Phyllis Wojtusik, RN. “This session focuses on how nursing facility teams can use live data-driven insights to identify those signals sooner and turn them into meaningful, timely interventions – before risk escalates and outcomes are impacted.”

Dr. Kimmel emphasized how live analytics support effective care delivery at the clinical level: “In value‑based care environments, outcomes are shaped by how well teams anticipate and respond to change over time. Access to live clinical insights allows the care teams we serve to prioritize more effectively, align across disciplines, and stay focused on patients most at risk – rather than responding after avoidable events occur.”

Attendees of PALTC26 are encouraged to join Real Time and Provider Partners for real‑world examples and practical strategies that Medical Directors and facility care teams can apply immediately to strengthen care delivery and performance. We look forward to seeing you there!
