Included Health Launches Included Health Plans Following Firefly Health Acquisition

Clinically-integrated health plans give self-insured employers flexible pathways to overhaul healthcare economics without shifting financial burdens onto employees

SAN FRANCISCO & WATERTOWN, Mass.--(BUSINESS WIRE)--Included Health, a new kind of healthcare company delivering personalized all-in-one healthcare to millions of people nationwide, today announced the launch of Included Health Plans for the January 1, 2028 plan year, offering self-insured employers a true alternative and challenger to legacy insurer-led models.

"Included Health Plans are about forging the path companies need to overhaul the economics of healthcare without having to cost-shift to employees.”

Through nationwide care delivery, personalized support, clinical guidance, proven network optimization, and benefit design, Included Health Plans empower members to choose high-quality, cost-effective care. By making better decisions easier across the healthcare journey, the plans deliver total cost of care savings ranging from 6% to 15% with sustained savings over time — without compromising member experience or clinical outcomes, or shifting costs to employees.

“Healthcare costs have moved from being a tolerable burden to becoming a threat that will sink businesses. Incumbent models don’t work — not for employers, not for people,” said Owen Tripp, co-founder and CEO of Included Health. “Included Health Plans are about forging the path companies need to overhaul the economics of healthcare without having to cost-shift to employees. Included Health Plans give employers a clear value proposition and flexibility: Supercharge your existing network, or build a supercharged network to make the highest-quality care the easiest choice. In either case, engagement, trust, and better health are the result — along with meaningful cost savings. We’ve seen the approach play out over the past two years with proven results.”

Included Health Plans integrate clinical care directly into the health plan experience, giving members convenient access to advanced primary care, behavioral health, acute care, and expert medical opinions, backed by an in-house, nationwide network of more than 1,000 virtual care providers. In addition, members have access to more than 2,000 Firefly Nearby in-person care partners, as well as specialty care from Carrum Health, Oshi Health, Pomelo Care, and Hinge Health. The integration of clinical care and system-wide navigation guides members toward quality and rewards cost-effective network utilization.

“For years, healthcare has been trapped in a cycle of escalating costs and diminishing returns,” said Fay Rotenberg, former CEO of Firefly Health and now president of health plan at Included Health. “When we founded Firefly, our thesis was to invert that broken equation and build half-priced healthcare that’s twice as good. Firefly built the full-stack model to realize that vision, bringing care delivery, network, navigation, and coverage together into one integrated plan experience. Now that Firefly is embedded within Included Health, we can scale that model and its outcomes more quickly and give employers a sustainable path to a lower total cost of care while actually elevating the member experience."

Flexible employer offerings built for outcomes and savings impact

Included Health Plans meet employers where they are through two proven plans designed to deliver total cost of care savings ranging from 6% to 15% with sustained savings over time. Employers can choose a comprehensive configuration designed to maximize impact on member experience, outcomes, and savings, or a flexible configuration designed to work alongside existing carrier networks and third-party administrators (TPAs). The comprehensive plan builds on and scales the Firefly Health plan already in market, which has demonstrated total cost of care savings of over 15%, while delivering 90% member satisfaction and proven results across preventive care, chronic disease management, and complex care. Both plans provide access to multiple national provider networks, dynamic copay-focused benefit designs, and full TPA functionality and integrations.

Included Health Plans are available to self-insured employers for a January 1, 2028 plan go-live date. For more details on Included Health Plans go to: includedhealth.com/alternative.
