New Digital Maternal Health Hub Aims to Close Critical Gaps in Maternal Health Care Across Pennsylvania

New digital platform connects mothers to personalized, comprehensive health resources and community services through a single digital entry point, funded by the Pennsylvania Department of Health, Bureau of Family Health.

AUDUBON, Pa.--(BUSINESS WIRE)--The Lincoln Center for Family and Youth (TLC), a nonprofit leader in community-based mental health services, has partnered with Greenspace Health to develop Bloom, a centralized digital platform designed to streamline access to maternal health care and social determinants of health (SDoH) resources and services for underserved communities across Pennsylvania.

According to Brown University Health, only 15% of pregnant people with depression and anxiety get connected to care, with stigma, cost, and navigation barriers among the leading causes (Bublitz & Sharp, 2024). Bloom, the Maternal Health Hub, will serve as a "one-stop" digital entry point connecting pregnant and post-partum individuals to comprehensive maternal health resources, community referrals, and dedicated case management support. The platform will leverage Greenspace's Measurement-Based Care (MBC) technology to collect user-reported outcome measures, guide personalized care recommendations, helping ensure that every individual's unique challenges, demographics, and location inform the recommended support pathways they receive.

The initiative initially targets Montgomery and Lancaster counties, where poverty rates exceed the statewide average, rates of preterm birth and low birth weight are disproportionately high among mothers of color, and access to first-trimester prenatal care lags significantly behind state averages (Pennsylvania Department of Health, 2022). Bloom addresses these barriers directly through a free, confidential, and user-friendly platform built in partnership with maternal health experts and community stakeholders. Within the two-year project period, TLC and Greenspace plan to expand the platform to Wyoming and Sullivan Counties.

Greenspace's Population Health Hubs have demonstrated clear, measurable impacts, achieving a 30-day user retention rate of 43% compared to a 3.3% industry average (Baumel et al., 2019), with users showing an average symptom improvement of 19%. Bloom directly combines digital access, outcome measurement, and human-centered case management in a single, easy-to-navigate platform.
