Evidently Unveils Omni — The Missing Connection Between Patient Complexity, Care Delivery, and Reimbursement
Announced at Becker's Health IT + Digital Health + RCM Conference 2026 in Chicago, the foundation of every Evidently interaction over the last three years, and a new way to interact with the patient chart: Skills that adapt to any user's specialty and role, from bedside to revenue cycle management.
PALO ALTO, Calif.--(BUSINESS WIRE)--Evidently, the leading Clinical Data Intelligence platform, today unveiled Omni, the foundation of every Evidently interaction over the last three years and the missing connection between patient complexity, care delivery, and reimbursement. Omni creates a real-time picture of every patient's lifetime of medical history and complexity by reading and understanding the entire longitudinal patient record, including the data trapped in hard to reach scanned documents, faxes, imaging, and in external systems like Care Everywhere and CommonWell. It then surfaces this full lifetime medical history and makes it available to everyone who interacts with the chart. Alongside Omni, Evidently launched Skills: a new way to interact with the patient chart that adapts to any user's specialty and role, with voice input on every surface.
"You can throw technology at a problem all day long. The question is, is it adopted? Our utilization went up 74%, and 83% of our providers said, 'Don't take this away.'" — Dr. David Marcozzi, Chief Clinical Officer, University of Maryland Medical Center

The Only Platform That Delivers Clinical and Financial Impact, Together, from a Full Picture of Patient Complexity
The average patient's medical record is now hundreds of pages long. And massive value is buried in these hundreds of pages: both in reimbursement that reflects the true complexity of care, and most importantly, in grounding care delivery in an informed picture of the patient's lifetime of medical history. Meanwhile, medical knowledge doubles every 75 days, trapping clinicians between two rising mountains of information. The system asks clinicians to synthesize it themselves, across hundreds of pages, dozens of sources, and years of encounters, while the knowledge needed to interpret it accelerates away from them and the time allotted to process it is shrinking. It's an impossible task.
Clinical and financial impact are treated as separate, often competing challenges because no system has been able to recreate the complete picture of the patient. Without that picture, health systems address each department need with a different tool, each working from a different, incomplete slice of the patient chart.
Omni recreates a real-time picture of every patient's lifetime of medical history and complexity, built from the entire longitudinal patient record: clinical notes, scanned documents, imaging reports, faxes, outside records, and Care Everywhere documents exchanged across institutions. As the chart changes, the picture changes. Every condition, every medication, every finding, mapped through a proprietary medical knowledge graph, reconciled across contradictions, and traceable to its source documentation. Unlike approaches that search for keywords, Omni understands what's clinically connected whether or not anyone thought to ask. Learn more at evidently.com/omni.
Skills: A New Way to Interact with the Patient Chart
The new experience incorporates three years of learning what clinicians need in real clinical settings, refined over months of testing with thousands of clinicians. It's built around Skills: a new way to interact with the patient chart. Skills are a layer on top of Omni that adapts to a user's role, including clinical and non-clinical roles, so they can build specific prompts of the chart, draft notes, and get exactly what they need. Thousands of distinct Skills are already in use across the platform.
The question in clinical AI is no longer about decision automation or where to set the automation threshold. Most clinical workflows have too many nuances and depend too heavily on clinician judgment. The question is how to empower clinicians' own reasoning. Skills automate the low-level information synthesis that consumes most of a workflow, so clinicians can focus on the high-level decisions that require their expertise. Seamlessly integrated as an activity tab and into the workflows clinicians already depend on daily.
Skills can be created and applied at the patient level, but also at panel and population levels, empowering population health and revenue cycle management teams with an unprecedented level of flexibility and insight.
Skills allow drafting of any clinical document grounded in the complete patient record, from admit and progress notes, to denial appeals, through to less-standard things a clinician drafts: an explanation for a patient's family, or Make-A-Wish applications.
And for the questions that don't fit a repeatable Skill: just Ask Evidently, free-form chart chat built on Omni's full-patient picture. Voice input on every surface.
As one hospitalist put it: "Evidently is my seatbelt. I don't drive without it."
Proven Financial and Clinical Impact, Three Years Running
The Omni foundation powering Skills is well-proven, driving the past three years of clinical and financial outcomes at leading health systems: one of the largest single-tool experience improvements ever measured in KLAS history (+31.7 points), with 9 out of 10 clinicians reporting better patient care, 15 minutes saved per admission on average, and 60% time savings on new patient workup. In inpatient settings, customers have observed 0.1 CMI improvement and a median of 4.5 new quality-impacting conditions identified per admit note, independently validated across multiple institutions. In outpatient settings, customers have observed up to a 0.11 per-member-per-year accepted RAF lift, and 6x return on investment in the first 90 days of deployment.
Customers also observe an overall 99.5% accuracy in independent clinical review, with a 0.7% omission rate and no safety-impacting errors. The remaining 0.5% were found to be secondary technical and attribution issues with no impact on care decisions.
"The upside to Evidently was clearly taking better care of patients. But the thing that enabled us to justify it financially was the impact on our case mix index, which went up by about 0.1. Being able to document those CCs and MCCs has tremendous value to the organization, which far exceeds the cost of the product. That made it an easy win." — Chief Health Information Officer at a leading academic medical center
Unprecedented AI Scale and Adoption
At two academic health systems where Evidently runs alongside leading ambient listening platforms, Evidently reaches roughly twice the clinician adoption. On a per-institution basis, Evidently runs more AI operations per month than any other clinical AI provider, ambient or EHR-native. Beyond core capabilities, clinicians and operational leaders actively extend the platform, authoring custom, localized workflows that directly address their health system's unique care delivery needs. Adoption spans the entire health system, from hospitalists and surgeons to social workers, pharmacists, and revenue cycle teams, across inpatient, outpatient, and emergency settings.

About Evidently
Evidently is the leading Clinical Data Intelligence platform, bringing the entire longitudinal patient history back into the EHR workflows clinicians and revenue teams rely on every day, with every insight linked back to its source. That includes the scanned documents, faxes, outside records, imaging reports, and data in external systems like Care Everywhere and CommonWell that the EHR was never built to read. Allina Health, UNC Health, University of Iowa Health Care, and other leading health systems across the United States use Evidently across inpatient, outpatient, and emergency settings, with peer-reviewed accuracy and published clinical and financial outcomes. Evidently is headquartered in Palo Alto, CA. For more information, visit evidently.com.
