Humana Subsidiary iCare Selected for Family Care and Family Care Partnership Contracts Across Southeast Wisconsin

Independent Care Health Plan expands service area to all seven counties in Wisconsin's greater southeast region, ensuring continuity for existing members and new access to high-quality long-term care services

LOUISVILLE, Ky.--(BUSINESS WIRE)--Independent Care Health Plan (iCare), a Humana subsidiary, has been selected by the Wisconsin Department of Health Services to serve both the Family Care and Family Care Partnership programs in the greater southeast region. This award expands iCare's footprint to all seven counties in the service area and reinforces its leadership in delivering high-quality managed long-term services and supports.

The intent to award marks iCare's third consecutive Wisconsin geographic win, with the organization earning the highest overall evaluation score among awardees for both Family Care and Family Care Partnership. With this pending contract, iCare will now offer Family Care, branded Inclusa, in all 72 counties, while expanding access to additional individuals in Kenosha, Ozaukee, Racine, Sheboygan, Walworth, Washington, Waukesha to Family Care Partnership.

"For the members who trust us with their care today, our selection represents continuity and stability," said Kiva Gittings Graves, CEO of iCare. "Members will continue working with the same dedicated care teams, care managers and community partners they rely on every day. At the same time, more Wisconsinites will gain access to a plan that has an excellent track record in delivering long-term services and supports. Our mission remains simple: helping people live independently where they choose and achieve their best quality of life."

Family Care and Family Care Partnership help older adults and adults with disabilities access long-term care services and supports that enable them to remain independent and connected to their communities. Through personalized care management, community-based services and strong provider partnerships, iCare works to improve health outcomes while supporting member choice and independence.

The intent to award reflects iCare's longstanding commitment to the communities it serves and is a testament to its strong relationships with providers and community partners throughout the Southeast region. iCare’s associates demonstrate this commitment at work every day and in their off time. For example, in December 2025, iCare staff sponsored and volunteered at the Racine Kenosha Community Action Agency's holiday meal box distribution, helping provide 235 meal boxes to local families during the winter season.

"Being part of the communities we serve is central to who we are," Graves said. "Whether it's helping members navigate their health care or supporting local organizations that address social needs, we are committed to improving lives across southeastern Wisconsin."

This expansion positions iCare for continued growth while strengthening Humana's presence in Wisconsin's managed long-term care market. To learn more about iCare, visit www.icarehealthplan.org. To learn more about Inclusa, visit www.inclusa.org.
