The recognition, awarded as part of the KLAS K2 Collaborative, highlights a multi-organization effort involving Veradigm, a national health plan serving 27 million members, and Raleigh Durham Medical Group, an independent provider group in North Carolina.

Using the Veradigm Payer Insights (VPI) platform, the initiative embedded SDOH-triggered clinical reminders within the EHR at the point of care and connected providers to community resource referral networks in real time. The project produced measurable improvement in care gap closure among members with identified social needs and established a replicable, FHIR-aligned model for payer-provider interoperability at scale.

Making SDOH Data Actionable at the Point of Care
The collaboration addressed a persistent challenge in value-based care: payer-derived SDOH data rarely reaches the clinician at the moment it is most useful. Through the VPI platform, providers received structured social risk alerts within their existing EHR workflow — no additional tools or portals required. When a patient with an identified social need was seen, clinicians were surfaced relevant information and connected in real time to community-based referral networks.
A Standards-Based Model Built for Scale
Built on FHIR and aligned to Da Vinci guidance, the VPI framework is designed to be payer-agnostic and portable across EHR systems. The architecture reduces implementation burden for health plans and provider organizations, enabling faster expansion to additional payers and clinical settings without requiring custom integrations.
Key outcomes from the initiative, as reported in KLAS 2026 Points of Light Case Study 24, include improved care gap closure rates among members with identified social needs, increased SDOH awareness and engagement at the point of care, and a documented referral pathway connecting patients to community resources in real time.
