Innovaccer has been accepted across both Early Cardio-Kidney-Metabolic (eCKM) and Cardio-Kidney-Metabolic (CKM) tracks, spanning hypertension, diabetes, chronic kidney disease (CKD) and cardiovascular disease conditions affecting more than two-thirds of all Medicare beneficiaries.

Innovaccer's ACCESS participation model is designed to be zero-risk for health system partners. Organizations continue billing Medicare fee-for-service without disruption, while Innovaccer delivers technology-enabled care, and takes on outcome risk directly for the specified chronic conditions. CMS pays Innovaccer; health systems earn new referral and co-management revenue without absorbing new infrastructure or compliance obligations. For ACO participants, the model is complementary by design as ACCESS patients remain attributed to the health system's ACO, and program spend is excluded from ACO benchmarks for the first 18 months entirely.

Innovaccer also plans to expand into the Musculoskeletal and Behavioral Health tracks, positioning itself as a comprehensive chronic care management solution across all four ACCESS clinical tracks.
