Innovaccer Launches a Social and Community Health Information Exchange for Whole-Person, Community-Centered Care

The platform by Innovaccer unifies cross-sector data and will leverage AI to help state and county agencies deliver coordinated, consent-driven community care

SAN FRANCISCO--(BUSINESS WIRE)--Innovaccer Inc., a leading healthcare AI company, today announced the public launch of the Social and Community Health Information Exchange (SHIE), a unified, AI-powered platform designed to integrate fragmented data across health, housing, behavioral health, criminal justice, and social service systems. SHIE is custom-built to support county and state-level agencies by enabling coordinated, and consent-driven whole-person care through a compliance-first infrastructure.

Public sector agencies struggle with siloed systems, manual processes, and limited visibility across programs, resulting in delayed care, duplicated efforts, increased costs, and compliance risks. Innovaccer’s SHIE addresses these challenges by consolidating data from multiple sectors into a single platform, eliminating the barriers to cross-agency collaboration.

SHIE features strong safeguards for security and privacy, flexible integration options that streamline data exchange, and seamless connectivity across systems and departments. At its foundation, SHIE is built on Innovaccer’s unified data platform that drives interoperability, analytics, and intelligent workflows. This foundation makes it possible to deploy agentic AI responsibly, ensuring insights are actionable, secure, and compliant across the healthcare and social service ecosystem. Additionally, SHIE leverages data-driven analytics and dashboards to uncover disparities, monitor operational performance, ensure consent oversight, and identify gaps in care and need at a community level.

By creating comprehensive longitudinal records encompassing clinical, social, housing, and justice information, SHIE equips frontline professionals, including case managers, social workers, and public health nurses, and agency staff with real-time, complete insights. This enables more effective coordination of care and reduces operational blind spots.

With granular privacy and consent segmentation and standards-based interoperability, SHIE builds on what Community Information Exchanges (CIEs) started, that is, community coordination to provide system-level infrastructure for operations, analytics, and whole-person care. It will deliver AI-powered insights tailored to housing, mental health, and justice programs and supports alignment with statewide initiatives such as CalAIM, offering scalable and modular infrastructure for lasting community impact.

“We built SHIE because communities deserve better than fragmented systems and manual processes. Public agencies need modern tools that not only safeguard data but also unlock the power of AI to reveal gaps and guide interventions. With SHIE, we’re enabling a future where every agency can see the full picture and collaborate seamlessly across sectors. That’s how we create healthier, more resilient communities together,” said Abhinav Shashank, Co-founder and CEO of Innovaccer.

Innovaccer’s Social and Community Health Information Exchange is now available for state and county departments and public health agencies across the United States.
