Connecticut Children’s Care Network Teams Up with Innovaccer to Power Population Health and Improve Quality Outcomes

The pediatrician-led clinically integrated network will implement Innovaccer’s Data Platform, Population Health Analytics, and Quality Reporting solutions to deliver value-based care to over 140,000 pediatric lives.

SAN FRANCISCO--(BUSINESS WIRE)--Innovaccer Inc., a leading healthcare AI company, today announced a strategic partnership with Connecticut Children’s Care Network, a clinically integrated, pediatrician-led organization serving over 140,000 pediatric lives across Connecticut. Connecticut Children’s Care Network will deploy Innovaccer’s Data Activation Platform, Population Health Analytics, and Quality Reporting solutions to make data more actionable, improving care coordination, reporting, and outcomes for children and families.

Connecticut Children’s Care Network includes over 1,450 providers across 44 practice sites, unifying the nationally ranked Connecticut Children’s health system, its subspecialists, and a network of community physicians. The Network operates multiple value-based care contracts with both commercial payers and the state of Connecticut, making data quality and analytics core to its mission of delivering high-performing, coordinated pediatric care.

To support Connecticut Children’s Care Network in advancing its population health and value-based care goals, Innovaccer is rolling out its solutions in a thoughtfully phased approach. In the first phase, Innovaccer is focused on building a strong data foundation by deploying its Data Activation Platform (DAP) to integrate claims data from key payers, offering a unified view of patient information.

Alongside this, Connecticut Children’s Care Network will gain access to InGraph, an advanced analytics and reporting tool designed to help care teams track quality, cost, and attribution metrics across top payer arrangements. Complementing these capabilities, P360 will provide comprehensive performance insights for physicians, allowing them to better understand provider behavior and manage variation in care delivery.

The final wave of deployment will introduce Supplemental Data Reporting, allowing Connecticut Children’s Care Network to close quality gaps and streamline submissions across multiple payers. Together, these solutions will empower the clinically integrated network to move to proactive reporting, insight-driven decision-making, improving pediatric outcomes while supporting its transition to value-based care at scale.
