Connecticut Children’s Care Network will deploy Innovaccer’s Data Activation Platform, Population Health Analytics, and Quality Reporting solutions to make data more actionable, improving care coordination, reporting, and outcomes for children and families.

Connecticut Children’s Care Network includes over 1,450 providers across 44 practice sites, unifying the nationally ranked Connecticut Children’s health system, its subspecialists, and a network of community physicians. The Network operates multiple value-based care contracts with both commercial payers and the state of Connecticut, making data quality and analytics core to its mission of delivering high-performing, coordinated pediatric care.
