Innovaccer Partners with Primary Care Collaborative to Release the 2024 Evidence Report: Primary Care Drives MSSP ACOs Success

The report shows primary care-centric ACOs generate 2.4x higher savings through data integration.

SAN FRANCISCO--(BUSINESS WIRE)--Today, Innovaccer Inc. and PCC announced the release of the Primary Care Collaborative's (PCC) 2024 Evidence Report, "Primary Care: The MVP of MSSP." The report focuses on how primary care-centric ACOs generated better savings and delivered better care within the Medicare Shared Savings Program (MSSP). Between 2017 and 2022, high primary care-centric ACOs delivered 2.4 times higher savings compared to low primary care-centric ACOs.

The report highlights how organizations achieve better outcomes by leveraging unified patient health data. Among the featured case studies is the Community Health Provider Alliance (CHPA), which achieved a 10% savings rate by integrating data from over 15 sources. This approach enabled disease-specific learning labs and quality improvement initiatives, optimizing clinical strategies across its network of 20 community health centers. Recently, CHPA’s parent organization, Colorado Community Managed Care Network, partnered with Innovaccer to deploy the Data Activation Platform and population health analytics solution, turning fragmented data into actionable insights to drive success in value-based care arrangements.
