Medical Home Network, NeueHealth Help FQHCs Achieve $10.1M+ Savings

Medical Home Network (MHN) provider partners participating in NeueHealth Premier ACO achieved $10.1 million in gross savings and a 100% quality score in the 2022 performance year, according to data recently released from the Centers for Medicare and Medicaid Services (CMS). These results were achieved under the Global and Professional Direct Contracting Model, now known as the ACO Realizing Equity, Access, and Community Health (REACH) Model, which included 20 FQHCs across Ohio, Missouri and Illinois working to improve care for approximately 10,000 Medicare beneficiaries.

MHN is a leading care enablement partner for safety net providers. MHN and its provider partners participating in the NeueHealth (NYSE: BHG) ACO, worked together to transform care for patients and excel in value-based care.

Participating providers used MHN’s team-based care model. MHN supported the FQHCs in hiring and training care coordinators and care managers who provide culturally tailored, whole-person care. Participants were also supported by MHN’s proprietary AI-enabled risk assessments and stratification inclusive of social drivers of health and clinical needs, as well as performance analytics to track care gaps, reduce ED visits and inpatient utilization.
