HealthPartners unveils dramatic advance in care convenience and affordability

Hybrid care offers convenient, connected experiences to consumers and lowers costs by up to 15% for employers

MINNEAPOLIS--(BUSINESS WIRE)--HealthPartners today announced it is launching a ground-breaking hybrid preventive care experience designed to make health screenings, immunizations, and other preventive, routine care simpler and more affordable.

Hybrid preventive care is the latest innovation we are delivering to the market.”

How it works

Hybrid preventive care will leverage the organization’s team of clinicians, 50+ primary care clinics, which are among the highest performing clinics in the state for quality and total cost of care, and its 24/7 online clinic, Virtuwell, to deliver a highly-coordinated preventive care plan virtually and make accessing in-person services more efficient when they’re needed.

The experience is supported by clinicians, algorithms and augmented intelligence to develop individualized preventive care plans for each patient. First, patients will complete a digital smart assessment about their current health status, medical history, mental health and more. A nurse practitioner will review the assessment, including other available electronic health data and claims information, to determine preventive care needs. Then they will provide a plan and recommend any additional care, such as mammograms, colonoscopies, immunizations or lab tests. Patients will also have an option for counseling and coaching by phone.

Through the hybrid experience, patients can find the most convenient appointments available for any follow-up support they need – whether it’s in-person or virtual. It will also clearly define what’s considered preventive care, helping people better understand their health plan coverage.

Product can save employers up to 15% on premiums

While hybrid preventive care will be available to anyone regardless of insurance, the experience is optimized for those with HealthPartners plans, including its high-performing Select network, which can save employers up to 15% on premiums compared to broad network insurance options. HealthPartners members who use hybrid preventive care will receive an enhanced personalized experience that includes communications specific to their preventive care plans and more convenient access to other condition support resources through their plan like myStrength, Omada and more.

Hybrid preventive care will be an option available starting January 2024. HealthPartners will continue to expand this market-leading, innovative hybrid care approach for other specialties.
