RubiconMD Launches Premier Clinical Pharmacy Platform RubiconRx

NEW YORK--()--RubiconMD, the leading eConsult provider, announced the launch of its new pharmacy offering, RubiconRx, designed to bridge the gap between primary care and clinical pharmacy and provide patients access to the medication support they need. The medication management platform embeds pharmacists within the care team to drive improved clinical outcomes and reduce costs.

Of these costs, 71% are due to medication therapy problems that could be mitigated each year by improving prescriber decision-making.1 RubiconRx aims to break this cycle by creating a space for Primary Care Physicians and Pharmacists to work together.

RubiconRx leverages fill data, board-certified clinical pharmacists, and customized drug regimen recommendations to achieve better health outcomes and reduce total cost of care. Once the clinician enrolls in the RubiconRx program, the clinical team will proactively triage high-risk patients that can benefit from an eConsult or Comprehensive Medication Review (CMR). If a CMR is determined, a Clinical Pharmacist will conduct a telepharmacy review with the patient and then provide the clinic with patient-specific recommendations.
