SALT LAKE CITY--(BUSINESS WIRE)--ClientTrack, Inc. announced today that the Los Angeles County Board of Supervisors approved a contract with ClientTrack as the case management solution for the Department of Health Services’ (DHS’) innovative Housing for Health (HFH) program. Los Angeles County is the largest county by population in the United States and has the second highest number of people experiencing homelessness in the country.
The HFH program strives to end homelessness in Los Angeles County, reduce disproportionate use of expensive healthcare resources and improve health outcomes for vulnerable populations. According to a 2013 report by the Economic Roundtable, providing supportive housing to frequent users of Los Angeles County systems led to an average 72% decrease in health care costs, from $58,962 to $16,474 annually.
For more than 20 years, ClientTrack case management has played a critical role in helping community and government organizations improve administrative operations to more effectively serve individuals in their community. HFH will use ClientTrack to increase program efficiency, streamline the enrollment process, reduce wait times, and collaborate across program departments and referral services. ClientTrack will help HFH to enroll, qualify and place homeless individuals in housing. The system will match eligible clients to available housing units, and also help HFH to coordinate healthcare and community services to further drive the goal of long-term housing stability for these individuals.
