The case study, titled "How Medication Care Management Transforms Heart Failure Care and Cuts Costs in Rural Wisconsin Community," details the results of a collaboration between Security Health Plan and PlanPlus by Curant Health. The study examines how proactive medication care management, predictive analytics, and longitudinal clinical engagement improved medication adherence, reduced hospital utilization, and generated substantial cost savings. In 2023, Security Health Plan (SHP) collaborated with PlanPlus by Curant Health to address rising Total Cost of Care (TCOC) among members diagnosed with Congestive Heart Failure (CHF). Heart failure remains one of the most resource-intensive chronic conditions in the United States, with costs often compounded in rural communities where barriers to care can negatively impact medication adherence and health outcomes.
To address these challenges, PlanPlus deployed its validated, proprietary Medication Care Management® (MCM®) program to proactively identify, resolve, and prevent medication-related and health-related issues among SHP's heart failure population. The program was designed to improve medication adherence, reduce avoidable hospital utilization, and lower downstream medical spending through relationship-based clinical engagement.
Year One Results
Across the intervention population, PlanPlus delivered an average per-member-per-month (PMPM) Total Cost of Care reduction of $585.98, driven primarily by reductions in medical spending associated with fewer hospitalizations and emergency department visits. The program generated an estimated 5:1 return on investment for Security Health Plan.
Additional results included:
- Medication adherence improved to 90.6%
- Hospital admissions decreased by 39.5%
- Emergency room visits decreased by 15.6%
- Medical PMPM costs decreased by $694.79
- Improved adherence and optimized therapy resulted in a modest pharmacy PMPM increase of $108.81
The study attributes these outcomes to a combination of predictive analytics, daily claims-based monitoring, and ongoing member engagement conducted through Curant Health's multidisciplinary Member Experience and Outcomes Team (MEOT).
