Innovaccer Launches Copilots and Agents for Care Management, Reducing Documentation Time by 28% and Boosting Patient Engagement by 20%

The new capabilities aim to significantly reduce the administrative burden on care teams, with early results showing a 28% reduction in documentation time and a 20% increase in patient engagement.

Care managers often spend hours each day attempting to connect with patients, averaging three call attempts before successfully reaching a patient and as many as 10–12 attempts in Medicaid populations. These interactions are further burdened by the need to leave voicemails, follow up repeatedly, and document each outreach, taking an average of 20 additional minutes per patient.

Innovaccer’s AI Agents automate the initial phases of this workflow by initiating outreach through SMS and voice calls, sending reminders, handling scheduling tasks, reducing repetitive workload, and increasing efficiency. The Care Management Copilot automates high-burden tasks such as documentation, chart summarization, and care plan creation. By reducing manual effort, the solution enables care managers to spend more time engaging with patients and coordinating care, rather than navigating administrative work.
