Real Time Medical Systems Launches New Enhancements to Help Providers Achieve Value-based Care Initiatives

Company’s new solution enhancements enable providers to improve care coordination and collaboration through their patients’ post-acute care journey

BALTIMORE--(BUSINESS WIRE)--Real Time Medical Systems (Real Time), the industry-leading and KLAS Rated Interventional Analytics solution, announces the launch of three new solution enhancements geared towards driving customer success and helping providers succeed in value-based care (VBC).

Through the Real Time Connect API, providers and payers can easily access key patient data from their post-acute network EHRs, as well as leverage the unique clinical analysis that powers the company’s industry leading, KLAS Rated post-acute analytics solution. With this enhancement, critical data can be gained from diverse EHR platforms within a post-acute network to monitor chronic conditions, effectively manage patient’s care, and improve VBC outcomes. With Real Time’s proprietary interventional analytics data, including live readmission risk scoring, quality measures, and length of stay, providers can reduce hospitalizations and improve care transitions. This real-time data provides insights into post-acute networks to ensure patients are being placed at the right facility, with the right level of care at the right time, driving VBC initiatives and patient outcomes.

The Real Time Resident Dashboard is helping providers successfully identify a patient’s status with detailed, clinical information. The live resident dashboard delivers real-time patient overall status, helps to avoid readmissions, and provides the information necessary to improve transitions, lower unnecessary costs, and manage length of stay. This interactive dashboard includes basic patient details, vital signs, active diagnoses, clinical alerts, physician orders, lab results, quality measures, and readmission risk scoring. Additionally, this tool can help post-acute facilities identify patients requiring further examination due to triggering of clinical alerts, keywords, and/or identified as high risk or moderate risk for readmission - without having to access the EHR. Assessing and utilizing the dashboard can be a great proponent in instilling VBC across the post-acute network.

With the Real Time Advance Care Planning (ACP) Alert, a measurable score utilizing clinical data from the EHR is provided, as well as risk factors exhibited after the personal severity index (PSI), or once the MDS is completed. The ACP identifier, including ACP alerts, ACP reports, and ACP tagging, provide a comprehensive view of the patient. This score includes metrics relating to functionality, as well as clinical and mood status. A score of 9 or greater helps providers identify residents who may be moving towards end of life and who may require changes to their current care plans and Advance Directives. This time sensitive information allows providers to initiate discussions with a resident and/or family, including a review of Code status and end-of life decisions to provide the patient with the best quality of life care and respect the patient’s choices while they are of sound mind.
