Collective Medical Introduces Enhanced Clinical Data Capabilities to Support Better Patient Outcomes

Collective partners with Kno2 to enable physician and care team members with high-value clinical insights at the point of care to inform care decisions

SALT LAKE CITY--()--Collective Medical today announced enhanced product functionality to supply care teams with previously difficult to source clinical data to drive better care outcomes. Connected by Kno2®, and working alongside its several health information exchange (HIE) partners across the country, Collective is now systematically incorporating this enhanced clinical data—such as Continuity of Care Documents (CCDs)—within the Collective platform to drive workflow automation, generate high-value insights at the point of care and power deep learning models and analytics that better identify and flag at-risk patients for care teams.

According to a study by Health Affairs, inadequate care coordination—including inadequate management of care transitions—is responsible for an estimated $25 to $45 billion in wasteful spending. Seamless care transitions and real-time visibility into patient activity are critically important for care teams to ensure the best possible patient outcome. Collective relies on and works collaboratively with HIEs to deliver sophisticated solutions that close care gaps while improving the overall patient and care team experience. By working with HIE partners across the country to leverage both their data infrastructure and enhanced services like data ingestion, structuring and normalization, Collective is able to generate and deliver to the point of care more robust clinical insights and analytics than otherwise feasible.

By leveraging Kno2’s ability to interact through modern, RESTful APIs with its connections, including Carequality, Collective’s new CCD-enabled functionality provides care teams with a way to discover relevant and previously difficult to attain clinical data, such as social history, discharge summaries and medications, problem lists and other helpful care insights. This data will further support care teams across settings to reduce emergency department utilization and optimize transitions of care—including transitions to post-acute settings.

Collective is currently implemented across 36 states and is proven to significantly impact care delivery and patient outcomes. For example, according to a report published by the Oregon Health Leadership Council, use of Collective’s platform contributed to a statewide reduction in potentially avoidable ED visits from patients with patterns of high utilization by 11.2 percent across 2018. The OHLC also reported that in the same time period, comorbid substance use disorder-related visits fell by four percent and visits within 90 days following an initial care guideline fell 31 percent.

In Virginia, where Collective supports the ConnectVirginia HIE to enable the state’s Emergency Department Care Coordination program, all 129 of the commonwealth’s hospitals, including all MCOs, are part of the Collective network. According to an article by EHR Intelligence, after the first year of the program, fatal opioid overdoses in Virginia decreased from 1,230 in 2017 to 1,213 in 2018. This marks the first decrease after five years of increases in opioid deaths in the commonwealth.
