Manifest MedEx Partners with NinePatch® to Launch Statewide Social Health Information Exchange for Whole Person Care in California

New exclusive collaboration integrates clinical, behavioral health, administrative, and social data within Manifest MedEx’s network to advance CalAIM

RIVERSIDE, Calif. & COLORADO SPRINGS, Colo.--(BUSINESS WIRE)--Manifest MedEx (MX), California’s largest nonprofit health information organization, and NinePatch®, a leading SaaS platform for Whole Person Health and Referral Information Exchange, announced a new partnership to launch a statewide social health information exchange (SHIE) platform that will combine social drivers of health (SDOH) information and tools, such as care assessments, program enrollment, closed-loop referrals, and consent management, with the more than 50 million longitudinal health records on MX’s health data network. MX’s new SHIE platform will be powered by NinePatch technology and made available to MX’s network of more than 140 hospitals, 19 health plans, 12 county health agencies, and 2,700 providers across the state.

Providers and health organizations, including community-based organizations (CBOs), that participate in the MX network will be able to appropriately and securely share an individual’s health history, social needs, and social services information, as permitted. MX’s integrated tools combine this information into one unified view, helping healthcare providers, social service providers, and Medi-Cal plans deliver Enhanced Care Management and Community Supports services under CalAIM more effectively.

Launched out of award-winning community work in Colorado, NinePatch focuses on building relationships across health and social care channels to promote whole person care. One large county in California’s Central Valley will serve as the first implementation for MX’s SHIE, advancing the region’s community data exchange strategy to support public health and CalAIM goals. The county plans to deploy the platform across county departments and contracted CBOs, enabling them to securely track, report, and share social data while maintaining strict permissions that limit CBO access to sensitive health data. The SHIE platform supports appropriate role-based access to enable coordination across multiple county departments and social services organizations.

Manifest MedEx will begin offering its SHIE platform to California county health agencies in 2026. MX is a designated qualified health information organization (QHIO) under California’s Data Exchange Framework (DxF) and among the first HIOs in the country to participate in TEFCA™, the Trusted Exchange Framework and Common Agreement™. In addition to its partnership with NinePatch for its SHIE, MX is also collaborating with Findhelp, a social care referral platform, to support seamless, secure, and appropriate access to SDOH data for participants in the MX network.
