SALT LAKE CITY--(BUSINESS WIRE)--Eccovia Solutions announced today that Community Health Partnership (CHP), a coalition of more than 25 provider organizations in southern Colorado, has launched ClientTrack™ – Eccovia Solutions’ industry-leading community care coordination platform – to help provide integrated care for members of its program, Community Care of Central Colorado. Community Care coordinates care and delivery of services for Health First Colorado (Colorado’s Medicaid Program).
Coordinating care across Community Care’s population requires use of a proven platform specifically designed for this purpose, which ClientTrack is able to provide. The platform will enable CHP to move forward their mission of improving access and controlling cost for the Health First program. Improving access to healthcare and associated resources is fundamental to providing the highest quality of care.
