HUNTINGTON BEACH, Calif.--(BUSINESS WIRE)--Landmark Health and its affiliated medical groups (Landmark), a pioneer and current industry leader of value-based, in-home comprehensive care for complex, chronic patients, recently shared key results of their clinical model as a result of extending care into the homes of thousands of patients over a multi-year period.
Landmark is uniquely positioned to share real-life outcomes versus the model predictions of newer entrants into this field. The results are based on a retrospective cohort study of 36,393 Landmark Medicare Advantage patients who were attributed to Landmark between January 1, 2016 and June 30, 2018.
Patients engaged with Landmark had a 26% lower risk of death1 compared to non-engaged eligible members over an identified sample period.
Even for patients who did not elect hospice, Landmark’s interventions (including a focus on palliative care at the end of life) are associated with a 20% reduction in costs1 in the last 12 months of life.
