Oak Street Health and Humana Expand Availability of Value-Based Care in Pennsylvania

Organizations Teaming Up to Offer a Patient-Centered Experience for Humana Medicare Advantage Members at Philadelphia-Area Clinics

PHILADELPHIA--()--Oak Street Health, a network of health centers providing primary care for Medicare patients, and leading health and well-being company Humana Inc. (NYSE: HUM) have entered into a value-based care network agreement in the Philadelphia area designed to offer a coordinated, patient-centered experience and help Humana Medicare Advantage members in Pennsylvania achieve better health.

The agreement aims to offer quality, cost-effective care to enhance both the patient experience and health outcomes by giving Humana Health Maintenance Organization (HMO) and Preferred Provider Organization (PPO) Medicare Advantage health plan members in Pennsylvania access to Oak Street Health’s first patient care centers on the East Coast, all located in Greater Philadelphia.

Oak Street Health currently serves patients at four Philadelphia clinics recently acquired from CityLife Neighborhood Clinics - at 1200 W. Godfrey Ave., 1010 W. Lehigh Ave., 37 S. 40th St., and 6232 Market St. - serving the Fern Rock, Lehigh Avenue, University City and Cobbs Creek neighborhoods.

In addition, Oak Street Health will have a total of three other locations in the city. Two are scheduled to open later this year in Philadelphia – at 5050 Parkside Ave. and 3621 Aramingo Ave. – serving residents of West Philadelphia, Aramingo and Port Richmond neighborhoods. The third, at 2401 W. Cheltenham Ave. in Wyncote, is now open to patients and serves residents in lower Montgomery County, as well as in the Mount Airy and Oak Lane neighborhoods.

Oak Street Health contracts with Medicare Advantage and Medicare Part D Health Plans, including numerous Humana Medicare Advantage plans. In addition to the Pennsylvania locations, Humana Medicare Advantage members have access to 40 other Oak Street Health clinics located in Illinois, Indiana, Michigan, and Ohio.

Oak Street Health treats people with Medicare and has shown significantly improved outcomes, providing quality care and reducing costly hospitalizations, according to a recent case study1. The company provides personalized experiences and a variety of individual resources for its patients, including a dedicated care team with a doctor or nurse practitioner, and community rooms at each center.
