MINNEAPOLIS--(BUSINESS WIRE)--Allina Health, Benedictine Health System (BHS) and Presbyterian Homes & Services (PHS) have agreed to work together to create a new generation of transitional care that will provide post-acute care patients with a clinically advanced yet soothing environment in which to transition from hospital to home. This collaboration brings together the expertise of organizations that provide hospital-based acute care with the experience of organizations skilled in managing in-patient rehabilitation and transitional care services to create a clinically-integrated, patient-centered transitional care experience for the communities it will serve.
Together the three organizations will create a consistent model and care philosophy for the future of transitional care with the goal of making branded centers available to franchise operators. Focused on hospitality, wellness and service delivered in a resort, spa-like environment; these centers will specialize in customized, high-quality, technologically advanced rehabilitation and recovery programs designed to reach optimal levels of individual health and ability. A smooth transition back to home is the goal for each guest.
The first two of these next-generation transitional care centers are in the planning stages. The first will be a BHS-managed center on the campus of Unity Hospital in Fridley, part of Allina Health. The next will be a PHS -managed center on the campus of Abbott Northwestern’s WestHealth, also part of Allina Health.
“As health care reform moves forward our communities will need new approaches to ensuring that after people leave the hospital they receive the care they need to make a successful transition back home and don’t end up back in the hospital,” said Kenneth H. Paulus, president and chief executive officer of Allina Health. “By bringing together the acute care and rehabilitation expertise of our three organizations, we will be able to forge new paths that ease the transition from hospital to home.”
“We are pleased to be partnering with two organizations - equally committed as BHS - to the three aims of health care reform: better health, better health care and lower cost,” said Dale Thompson, president and chief executive officer, Benedictine Health System. “By focusing our collective effort on the needs of the patients in these newly designed settings, we can enhance the care and healing of each person we have the opportunity to serve.”
“This innovative collaboration is designed to more effectively meet community needs now and in the future,” said Daniel Lindh, CEO of Presbyterian Homes & Services. “Our concept integrates services in new ways that will reduce unnecessary hospitalizations, return hospitalizations, and increase better transitions to home. Each partnering organization brings expertise and resources that together will result in better outcomes and an improved customer experience.”
