Amerigroup and CareMore Health Expand Partnership to Increase Access to High-Quality Healthcare

Amerigroup Adds CareMore to Its Medicare Advantage Provider Network

GRAND PRAIRIE, Texas--()--Amerigroup is committed to increasing consumer access to high-quality, affordable healthcare. As part of this commitment, Amerigroup and CareMore Health (CareMore) announced today that Amerigroup has added CareMore to its preferred list of care providers for all its Medicare Advantage plans. Starting January 1, 2019, CareMore’s care providers will be available in-network for these health plans, resulting in lower out-of-pocket costs for consumers compared to seeing a care provider outside of Amerigroup’s network.

CareMore’s innovative model of care employs common sense solutions that have been proven effective for improving health for people with complex health needs, including chronic diseases. Amerigroup Medicare members who select CareMore Health as their primary care provider will gain access to the CareMore Care Center and a team of health professionals including doctors, nurse practitioners, community health workers, pharmacists, and behavioral health specialists who work in unison to offer customized and integrated care. The new CareMore Health Care Center is located in downtown Fort Worth. In September, Amerigroup and CareMore Health announced the launch of their partnership in Texas to expand care options for Amerigroup Medicaid consumers.

Amerigroup currently offers a number of health plans that meet the needs of individuals eligible for Medicare. These plans include Amerivantage Classic (HMO), Amerivantage Dual Coordination (HMO SNP), and Amerivantage Dual Secure (HMO SNP), which are available in the Dallas-Fort Worth area.

These Amerigroup health plans offer a variety of benefits, which may include:

  • $0 copay annual dental cleaning and an annual allowance of up to $2,500 for dentures, fillings, and other dental services
  • $0 copay eye exam and an annual allowance of up to $400 for eyewear
  • Up to $3,000 hearing aid benefit
  • Up to 60 one-way trips to health-related destinations and/or places like church or the grocery store, when approved by a service coordinator
  • Up to $360 quarterly allowance for over-the-counter medications and health products
  • Quit smoking programs, including a coach and lozenges or patches
  • A personal emergency response system, which gives consumers the ability to maintain their independence and delay, or possibly eliminate, the need to utilize assisted living services
  • LiveHealth Online, a service that gives individuals access to medical professionals through a computer or mobile device
  • Medicare Community Resource Support, which is a telephone based program (supported by Social Workers) used to identify needs, leverage resources, and coordinate services
  • Membership to SilverSneakers, a fitness program, at no extra cost
  • Everyday Extras package, which provides a choice of one of the following services: 64 delivered healthy meals after hospital discharge; 60 one-way trips to a health-related destination; a personal health helper for up to 124 hours care, chores, and assistance with activities of daily living; assistive devices allowance of $500, such as a shower grab bar; up to 1 day a week of time at an adult day center; and alternative medicine through 24 acupuncture or therapeutic massage sessions.
