CarePort Health Expands with Key Hires on Product, Data Analytics, and Customer Success Teams

Leading post-acute outcomes solutions provider expands to drive continued innovation and enhance customer support

BOSTON--()--CarePort Health, a leading provider of care coordination software solutions, today announced three new hires to further the company’s growth. Sara Radkiewicz leads CarePort’s Product team, Thomas Martin is building a Post-Acute Data Analytics team, and Michael Ipekdjian has joined the Customer Success team. These new team members bring a wealth of diverse experience to the company and are expected to drive continued innovation for the entire CarePort suite of products, as well as enhance support for both acute and post-acute customers.

PRODUCT TEAM

As the Head of Product, Sara Radkiewicz is managing the team responsible for continuing to build and scale CarePort’s Guide, Connect, and Insight solutions. Sara is working closely with CarePort’s founder and CEO, Dr. Lissy Hu, to realize her vision of bridging the data gap between acute and post-acute care. Before joining the company, Sara led product strategy and execution at an early startup as the Head of Product at TVision Insights. She has previously led product and product management efforts at Localytics, Bluefin Labs, and Brightcove. Sara is collaborating with stakeholders across the company to further innovate the CarePort platform.

ANALYTICS TEAM

As the Director of Post-Acute Analytics, Thomas Martin is helping post-acute providers identify high-risk patients and manage populations with different levels of acuity. Tom’s team is also helping post-acute providers prepare for upcoming regulatory and payment model changes. Tom brings a wealth of specialized knowledge in this area of healthcare from his prior positions at the American Health Care Association and PointRight. He is already enthusiastically sharing his subject matter expertise on the CarePort blog.

CUSTOMER SUCCESS TEAM

As a Director of Customer Success, Michael Ipekdjian is focused on addressing the unique pain points and needs of CarePort’s customers. Michael will spend the remainder of this year and early next year traveling across the country to customer sites and meeting with CarePort clients in person. As a former bedside nurse and inpatient acute case manager, Michael possesses intimate knowledge of the challenges that accompany care transitions. He has also held multiple care management roles and has significant experience building value-based programs. Most recently, he served as the Director of Transitional Care Management at Holyoke Medical Center, where he brought on CarePort’s products to improve care coordination for the health system.
