The University of Chicago Medicine and Humana Sign Value-Based Care Agreement To Offer an Enhanced Patient Experience For Humana Medicare Advantage Members

CHICAGO & LOUISVILLE, Ky.--()--The University of Chicago Medicine (UChicago Medicine) and leading health and well-being company Humana Inc. (NYSE: HUM) have entered into a value-based agreement designed to offer a coordinated, patient-centered experience to help Humana Medicare Advantage members achieve their best health.

The agreement aligns two organizations with a shared commitment to improving the patient experience and the health outcomes of people in the Chicagoland area. The agreement will provide access to value-based care for Humana Health Maintenance Organization (HMO), Preferred Provider Organization (PPO), and Private Fee-for-Service (PFFS) health plan members seeking care at the University of Chicago Medical Center; UChicago Medicine outpatient care centers and clinics; and the University of Chicago Physicians Group practices.

The agreement with UChicago Medicine is part of Humana’s longstanding commitment to value-based care, which emphasizes:

  • More personal time with health professionals and personalized care that is tailored to each person’s unique health situation;
  • Access to proactive health screenings and programs that are focused on preventing illness;
  • Improved care for people living with chronic conditions with a focus on avoiding health complications;
  • Leveraging technologies, such as data analytics, that connect physicians and help them work as a team to coordinate care around the patient; and
