Altegra Health Launches My Advocate™

The New Face of Outreach Services Replaces Social Service Coordinators®

MIAMI LAKES, Fla.--()--Altegra Health, a leading provider of business, technology and consulting solutions that improve quality and financial performance for health plans and at-risk provider groups, today introduced “My Advocate” as the new brand for the company’s services that help healthcare organizations enhance member care and quality of life, replacing the former company name known as Social Service Coordinators®.

The combined suite of member engagement services, now called My Advocate™, is a complete, technology-enabled concierge outreach service provided through health plans for their members. It is designed to help Medicare Advantage, Managed Medicaid, and Commercial health plan members save money, receive needed assistance, and take better care of themselves. The goal of My Advocate is to help plans enhance member satisfaction, lower long-term care costs, improve Star Ratings, and optimize health outcomes.

The My Advocate outreach services provided by Altegra Health were previously known as Social Service Coordinators®. Under My Advocate, Altegra Health will continue to provide these services and other quality improvement and risk adjustment solutions to partners, including multi-regional Medicare Advantage, Managed Medicaid and Commercial plans, as well as regional and state-based organizations.

Since 2000, Altegra Health has provided assistance in applying for Medicare Savings Programs (MSPs) and Extra Help, primarily to “dual eligible” and low-income individuals. Additional services include community program enrollment assistance, which helps members with prescription and household utility expenses, as well as transportation, nutrition, healthcare premium and co-payment assistance. In addition, My Advocate will provide a wide variety of care management and other health education programs. These include physician appointment scheduling and reminders, education and lifestyle management for chronic diseases, medication adherence, and recertification services.
