BELLEVUE, Wash.--(BUSINESS WIRE)--Caradigm, a GE Healthcare company, today announced that MissouriHealth+ (MHP), a network of Community Health Centers serving diverse patient populations across the state of Missouri, has selected Caradigm’s enterprise population health management solution suite to improve the lives of its patients and deliver on value-based care contracts with payers.
Through the combination of Caradigm data analytics and care coordination solutions, MHP will bring together siloed data to help inform and automate care coordination and care management for assigned populations.
MHP includes a member-owned accountable care organization (ACO) with 24-member Community Health Centers managing nearly 210,000 lives under Medicaid, Medicare and commercial lines of business. By using Caradigm’s suite of population health solutions, including: Caradigm Intelligence Platform (CIP), Care Management (CM), Quality Improvement (QI), Risk Stratification (RS), Utilization & Financial Analytics (UFA), and Knowledge Hub (KH), MHP plans to improve patient outcomes, reduce total cost of care and maximize quality incentives.
