STRAND Platform Achieves Pharmacist eCare Plan Level 2 Capability

Continues to Empower Pharmacists as they Provide Medication Use Supports and other Enhanced Pharmacy Services for their Patients

EVANS, Ga.--()--Creative Pharmacist, the market leader in developing and supporting clinical services offered by community pharmacies, announced today that its STRAND® Platform has successfully achieved ‘Level 2 capability’ for the development and deployment of the Pharmacist eCare Plan initiative, a project supported by the Pharmacy Health Information Technology Collaborative that is being tested by Community Care of North Carolina (CCNC) to aid in the integration of community pharmacy patient encounters with care management and primary care delivery and information systems.

The Pharmacist eCare Plan is a shared document detailing a patient’s current medication regimen and health concerns, including drug therapy problems, therapeutic goals and medication support needs, in addition to the pharmacy's interventions and the patient's health outcomes. ‘Level 2 capability’ includes the transmission of this critical clinical information to be shared with other care team members. “Since we received notification of the successful completion of ‘Level 1 capability’, we’ve transmitted thousands of Pharmacist eCare Plans via our STRAND® clinical platform,” shared David Pope, PharmD, CDE, and Co-Founder of Creative Pharmacist. “We are excited about our role in empowering pharmacists to document clinical findings and send that information to their patient’s physician and other care team members.”

The STRAND® Platform is integrated with the industry’s top pharmacy management systems, including Computer-Rx, Pioneer Rx, Rx30, QS/1, Liberty Software, and VIP Pharmacy Systems and provides its eCare Plan technology for pharmacists using these systems, among others.
