Safety Net Connect Receives Association for Community Affiliated Plans (ACAP) Preferred Vendor Status

Health IT Innovator Selected for Proven Technology Solutions That Enhance Whole-Person Care Coordination for Safety Net Populations

NEWPORT BEACH, Calif.--()--Safety Net Connect (SNC), a leading provider of innovative web-based healthcare technology for organizations assisting underserved populations, today announced its selection as a Preferred Vendor by the Association for Community Affiliated Plans (ACAP), a national trade association representing 60 nonprofit safety net health plans in 29 states – serving over twenty million enrollees and representing nearly half of individuals enrolled in Medicaid plans.

The ACAP Preferred Vendor Program provides member plans with streamlined access to pre-qualified vendor organizations that understand the complex needs of safety net care providers. All Preferred Vendors must be sponsored by at least one ACAP plan prior to undergoing a rigorous board and plan representative approval process. SNC holds a long-standing partnership with ACAP member plans, L. A. Care Health Plan and Partnership Health Plan of California.

SNC’s premier products “Converge” and “Connect” are web-based care coordination solutions that have produced significant outcomes in safety net populations, including: increasing network adequacy with an 89% reduction in wait times for specialty care and a 60% reduction in inappropriate face-to-face specialty care visits; improving quality and closing gaps in care with a 25% reduction in 30-day hospital readmissions and a 130% increase in timely post-discharge follow-ups; and increasing efficiencies and cost savings – including year-one cost savings of $17.7m for an Illinois Medicaid health plan.
