‘Realizing the Value-Based Hospital’ Among Highlights at CEP America’s 40th Annual Partnership Meeting

CEP America Hospital Partnerships Recognized for Outstanding Achievement in Faster Emergency Care, Fewer Readmissions, Improved Case Mix Index and Increased Patient Satisfaction.

EMERYVILLE, Calif.--()--How hospitals can make the challenging shift from fee-for-service payment (FFS) models to be successful in a value-based care (VBC) system was one of the key questions addressed at CEP America’s 40th Annual Partnership Meeting this past September.

Held in Los Angeles, the three-day meeting was attended by more than 1,000 of the nation’s leading hospital and health system CEOs, administrators, physicians, nurses and other industry thought leaders. Speakers presented innovative strategies for managing increasing complexities in care and payment systems and insights into improving care as the healthcare industry nationwide faces its largest reform in modern history. Topics addressed included: improving operational efficiencies to provide value; integration of patient care across the acute care continuum; how to achieve medicine without walls; and creating a true team approach to patient care.

While fee-for-service using alternative payment models will remain for some time, the transition to value-based care is changing the ways in which patient care is going to be delivered in the future. To remain viable and relevant as healthcare reform unfolds, hospitals need to partner with physicians and nurses in new ways to improve outcomes and the patient experience, while at the same time achieving performance metrics greater than the standards today.

At the meeting, CEP America showcased best practices from over 100 unique hospitals and acknowledged partner hospitals with outstanding achievements in providing faster emergency care, reducing readmissions, improving Case Mix Index (CMI), and increasing patient satisfaction.

The award-winning practices include:

  • Practice of the Year - Adventist Health Central Valley Network, CA. Nine integrated practice locations across four practice lines: Emergency Department/Hospital Medicine/Skilled Nursing Facility/Ambulatory & Urgent Care.
    • For the first time, the Practice of the Year award is received by a network of integrated multispecialty practices. This network of physicians manages patients with acute care conditions who are hospitalized - including those needing skilled nursing care - and post discharge follow up, all coordinated by a local CEP America physician in the role of Transitions of Care Director. The network has significantly decreased average length of stay (9% at one hospital alone) and achieved the highest Case Mix Index (measure of the relative cost or resources needed to treat the mix of patients) in history.
  • Distinguished Practice - Memorial Hospital, Belleville, IL. Emergency Department/Hospital Medicine.
    • Among their many accomplishments, Memorial Hospital improved discharges before 11 a.m. by 70%; decreased readmission rates by 6%; reduced turnaround time to admission by 10%.
  • Distinguished Practice – Northwest Community Hospital, Arlington Heights, IL. Emergency Department.
    • Northwest Community Hospital reduced already low 1.89% left-without-being-seen rate to 0.59%; reduced turnaround time to discharge by 27%.
  • Distinguished Practice – Palomar Health/Pomerado Hospital, San Diego Metro Area, CA. Emergency Department/Hospital Medicine /Skilled Nursing Facility.
    • Palomar created a Patient Flow initiative, including a Centralized Patient Placement Center and Dashboard to manage the flow of patients across multiple sites and practice lines while reducing turnaround time to discharge by 50%.
  • Distinguished Practice – Rideout Memorial Hospital, Marysville, CA. Emergency Department.
    • Among CEP America partner sites, Rideout Memorial Hospital achieved the highest level of reductions in turnaround time to discharge; reduced time to provider from 18 minutes to five minutes; increased patient satisfaction by 5%.
