South Nassau Communities Hospital Selects RightCare to Help Reduce Avoidable Readmissions

Medical technology company enables premier New York hospital to offer an individualized approach to patient care and satisfy key DSRIP requirements

HORSHAM, Pa.--()--As part of its care transitions process to eliminate avoidable readmissions, minimize length of stay and boost the patient experience, South Nassau Communities Hospital has enlisted the help of RightCare.

RightCare’s full suite of software will be used by South Nassau’s case managers to better meet the individual care needs of patients through a unique, modular software solution that is designed to assess patient risk at the time of admission, ensure the appropriate care plan is in place and seamlessly transition patient information to post-acute care providers.
