Univita and SCL Health’s Integrated Care Delivery Joint Venture Featured in HealthLeaders Webinar

MIRAMAR, Fla.--()--Univita Health, along with its joint venture partner, SCL Health, was featured in a HealthLeaders webinar on forging partnerships across the care continuum. Dr. Rick Lopes, SCL Health’s Executive Vice President and Chief Population Health Officer, presented on how the joint venture is integrating the acute and post-acute delivery model to ensure patients receive high-quality, comprehensive and well-coordinated care from the time of admission through the home-recovery process.

“All major health care constituents are seeking opportunities to reduce cost and improve quality across the post-acute care continuum - a market that is estimated at over $60 billion,” said Lopes. “Comprehensive post-acute care management that includes longitudinal care management and coordination across settings of care will assume an increasingly important role in countering cost trends by mitigating readmissions, reducing acute care length of stay and bridging care delivery groups.”

The increasing demand of the joint venture’s services is due to its focus on integrating critical components of the acute and post-acute delivery model, aligning SCL Health’s acute care programs with Univita’s home-based care management to deliver a high-value continuum of care from hospital to home. In order to address the 80 percent of post-acute spend on home health and skilled nursing, the joint venture partnership recently acquired All Care Home Health, which serves as the home health care provider for the integrated care delivery model.

In order to support the increasing demand of the joint venture’s comprehensive approach to post-acute care management, the partnership has launched a website that illustrates the benefits of integrating care delivery for patients, caregivers, payers and providers.
