naviHealth and Highmark Inc. Announce Innovative Partnership to Enhance Post-Acute Care and Outcomes

naviHealth’s post-acute management program will help Highmark’s Medicare Advantage members in Pennsylvania navigate the post-acute continuum to achieve improved clinical outcomes

NASHVILLE, Tenn. & PITTSBURGH--()--naviHealth and Highmark Inc. have announced a three-year partnership to manage post-acute care services for Highmark’s Medicare Advantage members across Pennsylvania. The program will utilize naviHealth’s unmatched decision support technology, post-acute analytics platform and network of field-based clinicians to enable Highmark members to achieve improved outcomes following a hospital stay. With more than 285,000 members across Pennsylvania, Highmark is currently the largest Medicare Advantage plan in the state.
