ViaQuest Selects Netsmart CareManager™ as Coordinated Care Solution for Ohio Health Home

Solution Will Help ViaQuest Pursue Health Home Certification, Streamline Workflow and Offer Broader, More Comprehensive Services

OVERLAND PARK, Kan.--()--ViaQuest, Inc., a leading provider of behavioral health, home health, hospice, and developmental disabilities services, has chosen Netsmart CareManager™ as the care management and care coordination solution for its planned Health Home.

The company’s Clinical Services Division will utilize CareManager as part of the process to attain Health Home certification from the State of Ohio and to share approved health information with Health Home provider participants and the state’s proposed health information exchange.

Health Homes are comprised of teams of healthcare professionals responsible for coordinating care across multiple providers. By integrating primary care and behavioral healthcare, and coordinating care regardless of where patients receive it, Health Homes are designed to help improve quality of care and health outcomes while also bending the cost curve.

CareManager also facilitates care coordination by allowing providers within a Health Home to follow a shared care plan, exchange authorized health data, and track clinical quality measures and outcomes across the network.
