naviHealth and Cigna-HealthSpring Partner to Improve Patient Outcomes and Reduce Costs in Post-Acute Care

NASHVILLE, Tenn.--()--naviHealth and Cigna-HealthSpring, a Cigna company (NYSE:CI), today announced their collaboration in managing post-acute care services for approximately 100,000 Cigna-HealthSpring members in the Mid-Atlantic and Pennsylvania markets. The program will utilize naviHealth’s decision support technology and field-based care managers to deliver high-touch care services to Cigna-HealthSpring members receiving Skilled Nursing Facility (“SNF”) services.

With the program, naviHealth’s nurses and licensed therapists will engage and assess Cigna-HealthSpring members in the hospital to educate members and their caregivers on their options for post-acute care and rehabilitation. Post-discharge, naviHealth will work with patients and their care teams in skilled nursing facilities to continually update the patient’s care plan and monitor progression against their personalized goals.
