Cloud-based platform supports expanded patient-centered medical home initiative and development of an Accountable Care Organization

At Symphony’s core is an interactive clinical checklist system that provides decision support, downstream notifications and alerts, automated documentation, and real-time clinical performance reporting. The platform fits directly within existing HMG physician-developed care coordination workflows and employs a highly intuitive user interface that enables quick deployment with minimal training.
