BURLINGTON, Mass.--(BUSINESS WIRE)--Arcadia Healthcare Solutions, a leading provider of data-driven quality improvement solutions for health systems and health plans, is pleased to announce that longtime client Yakima Valley Farm Workers Clinic (YVFWC) has received Level 3 Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance (NCQA), a private, non-profit organization dedicated to improving health care quality. Level 3 recognition is the highest achievable recognition level for a medical group. The NCQA Patient-Centered Medical Home 2011 program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), American Osteopathic Association (AOA) and others in extension of the Physician Practice Connections Program. It was developed to assess whether physician practices are functioning as medical homes and recognize them for these efforts. The NCQA Patient-Centered Medical Home 2011 standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.

Through this strategic partnership, Arcadia was able to help YVFWC reduce their timeline for clinical and data transformation by 70 percent, reduce total cost by 30 percent and increase performance and quality measures by 22 percent on average.

Applying for PCMH recognition is a complex and time intensive process, often taking practices with limited staff and resources more than a year to complete. Arcadia supported this effort by providing on-the-ground coaching and the technology needed to deliver effective, efficient team-based care. Guided by Arcadia’s proprietary PCMH transformation project management tool, the PCMH Accelerator, YVFWC practices received in-practice coaching to define and implement policies and procedures based on NCQA guidelines.
