Ambience Healthcare Expands Chart-Aware Intelligence to Full Inpatient Workflow
Building on its chart awareness platform, Ambience now delivers clinical context across the full hospital stay, from admission through discharge
SAN FRANCISCO--(BUSINESS WIRE)--Ambience Healthcare, the leading intelligence platform for clinical documentation and revenue integrity, today announced a significant expansion of chart-aware intelligence across its inpatient platform. Ambience previously introduced chart awareness, enabling AI to read and reason over a patient's full record rather than the conversation from a single encounter.
With this expansion, chart-aware intelligence now covers every step of the hospital stay, from admission through discharge, synthesizing prior notes, labs, imaging, medications, vitals, and orders across every workflow. The result is documentation that reflects the complexity of a patient's care and a health system that captures the revenue it has earned.
Inpatient medicine is fundamentally different from the outpatient setting. Hospitalists manage 15 to 20 patients simultaneously, each with evolving diagnoses, shifting labs, and medication changes accumulating day over day. The cognitive load means clinicians often carry yesterday's note forward rather than synthesizing what changed overnight, and context gets lost between shifts, departments, and care teams. When documentation is incomplete, it cannot be translated into the right billable codes, care gaps emerge, and health systems are not appropriately compensated for the care they actually provided.
Most ambient AI was built for outpatient care, where a clinician enters a room, has a conversation, and the AI generates a note. In roughly half of all inpatient encounters there is no conversation to capture, and even when there is one, non-conversational data, including labs, medication changes, imaging, and prior diagnoses, is what actually drives documentation and care. Ambience's analysis found that 70% of clinically important inpatient diagnoses have no signal in the transcript. For the diagnoses that matter most for coding and reimbursement, that figure is even higher.
"Hospitalists have never had a tool built for how they actually work," said Dr. Michael Han, Chief Medical Officer at Ambience Healthcare. "The majority of what drives inpatient documentation never enters a conversation. By bringing chart-aware intelligence across every workflow, we can finally give hospitalists a complete picture of their patients, whether or not a recording is ever captured.”
With Patient Summary now live in the inpatient setting, chart awareness is embedded across the full inpatient arc:
- Patient Summary: Before the hospitalist enters the room, Ambience synthesizes the patient's longitudinal history, giving clinicians the specific context they need to orient quickly to patients.
- History and Physical (H&P) Notes: Generates the full H&P from the bedside interaction, if applicable, alongside chart data.
- Diagnoses: Surfaces missed conditions and precise ICD-10 codes with supporting evidence in real time, drawing from the bedside interaction, hospital problem list and months of chart data.
- Progress Notes: Builds each note from the prior day’s note together with the current bedside interaction (if one occurred) and real-time chart data. A progress note no longer depends on a recording or a copy forward from the prior day.
- Hospital Stay Summary: Maintains a running synthesis of the admission to support clean handoffs across the care team.
- Discharge Summary: Constructs a final discharge summary from the complete hospital stay in one click, with the option to enrich it via a recorded discharge conversation.
Together, these capabilities make Ambience the only inpatient platform with chart awareness across every step of a patient stay. In evaluations across 66 encounters at four health systems, chart-aware discharge summaries resolved 91% of information gaps that standard documentation had missed.
These chart-aware capabilities have driven measurable adoption at health systems that have deployed the full inpatient solution. At Saint Luke's Health System, utilization across H&P notes, progress notes, and discharge summaries exceeds 70%, with NPS scores rising 31 points. Nearly one in five Ambience employees is an active or trained clinician, embedded across product development, model training, and clinical validation. Building alongside clinicians is what makes it possible to build tools they actually trust and use.
When clinician notes reflect the full clinical picture, diagnoses are accurately captured, handoffs are cleaner, and clinicians spend less time on documentation. Most importantly, patients receive care grounded in the full context of their stay. To learn more, visit ambiencehealthcare.com.
