USMD Joins Catalyst Health Network Accountable Care Organization

Clinically Integrated Network Empowers Physicians and Patients to Improve North Texans’ Health and Well-Being

PLANO, Texas--()--Catalyst Health Network, one of the largest clinically integrated networks in the nation, has added USMD Health System to its expanding accountable care organization (ACO) network in the Dallas-Fort Worth metropolitan area. The addition of USMD to Catalyst Health Network will significantly increase the size of the network, bringing together 465 primary care providers (PCPs) in more than 140 locations that serve more than 750,000 patients across North Texas.

The Catalyst Health Network ACO is transforming communities’ health and well-being through enhanced physician-patient relationships based on stronger, more collaborative primary and preventive care. By deploying team-based care management and utilization strategies in conjunction with the PCP, care providers can ensure the patient receives the appropriate care in the appropriate setting for all their health care needs.
