MADISON, Wis.--(BUSINESS WIRE)--Anthem BlueCross and BlueShield (Anthem) and Inclusa are collaborating on a pilot program that will expand coordination services for Medicaid members throughout Wisconsin. The program is designed to improve care coordination, social services supports and better link members to community resources.
“We believe this pilot program will positively impact Medicaid members by enhancing our care management and social service coordination capabilities,” said Leon Lamoreaux, Anthem BlueCross BlueShield plan president. “By expanding our presence in rural parts of the state and connecting with members at locations that are convenient for them, we are confident that this pilot program will create a new standard of care and social service coordination for Medicaid SSI members.”
As part of the pilot program, Anthem and Inclusa collaborate to meet Medicaid SSI members in their regular environments including: in-home, community-based organizations, treatment centers and other convenient locations. Inclusa will focus on members in rural parts of the state while Anthem will focus on members in Milwaukee and the surrounding areas.
The pilot program will maximize access to health programs and improve quality of life outcomes for members with SSI eligibility.

The pilot program will run through the remainder of 2017.
