Community Medical Group and Anthem Blue Cross Blue Shield Expand Collaborative Agreement to Improve Health Outcomes

WALLINGFORD, Conn. & NEW HAVEN, Conn.--()--Anthem Blue Cross and Blue Shield in Connecticut (Anthem) and Community Medical Group (CMG), a physician-led independent practice association based in New Haven, today announced an agreement to provide expanded services to their shared patient population. CMG will build upon its existing capabilities and receive additional physician enablement tools from a separate Anthem company providing services directly to CMG to support CMG’s efforts in the Anthem value based program.

“Combined, CMG’s continued participation in Anthem’s value-based care program and these new services will optimize our ability to improve patient health and reduce the cost of care,” said Joseph Quaranta, M.D. President of CMG. “This agreement means unsurpassed care for patients treated by our independent physicians as we continue to deliver the right care at the right place at the right time.”

Physician enablement services involves the delivery of payor-agnostic tools and processes to help doctors close care gaps, utilize best practices for disease management and reach out to patients who are overdue for immunizations or prescription refills.

This separate enablement services agreement comes as CMG enters its fourth year of participation in Anthem’s value-based care program, called Enhanced Personal Health Care, that’s aimed at strengthening the relationship between physicians and patients by allowing them to meet each patient’s unique needs. Doctors who participate in Enhanced Personal Health Care work with Anthem to use data-driven insights to better promote the health of their patients, from identifying and managing high-risk patients to proactively reaching out to patients who have not received recommended preventative care.
