St. Luke’s University Health Network Goes Live With Caradigm Care Management to Support Bundled Payments for Care Improvement

Achieves Rapid Time to Value Through Improved Process Workflows, Data Sharing Across the Continuum

BELLEVUE, Wash.--()--Caradigm®, a leader in enterprise population health, today announced that St. Luke’s University Health Network (SLUHN) has implemented Caradigm Care Management software to support its participation in the Bundled Payments for Care Improvement (BPCI) program.

SLUHN is one of the largest BPCI participants in the country, having signed up for 44 of 48 available episode bundles. To better serve the needs of residents across the Lehigh Valley and to prepare for value-based care delivery, SLUHN required tools for care coordination that would span both inpatient and outpatient settings, as well as combine its claims data with clinical data from its Epic EHR system.

The central challenge for bundled healthcare services is to make care more coordinated and efficient across multiple settings and organizations. Organizations must bring together data from across the continuum and better collaborate with other providers in order to drive improved patient outcomes and eliminate unnecessary costs. Caradigm Care Management addresses these requirements by enabling multi-disciplinary care teams to share information and streamline workflows as patients move from acute to post-acute settings.
