Virginia Care Partners and Transcend Sign Agreement to Improve Health of Medicare Advantage Members

Physician-led network focal point of collaboration with Transcend

RICHMOND, Va.--()--Virginia Care Partners (VCP) has entered into an agreement with Transcend to provide population health management services through value-based care to 10,000 Richmond-area Medicare Advantage patients.

Transcend will work with more than 90 VCP primary care physicians, consulting with them in their offices to help track, analyze, and act on data about their Medicare Advantage patients’ health. Transcend’s support includes services that will help VCP physicians better identify and address opportunities to improve care and health outcomes.

VCP was formed in 2012 by physicians who shared a commitment to improving their patients’ health outcomes by applying best treatment practices across all care settings – in physician offices, hospitals and outpatient care settings. VCP works collaboratively to negotiate value-based care contracts that provide incentives for improved patient outcomes, adherence to care standards, increased preventive care and chronic disease management, and reduced cost.

“Because primary care physicians are expected to manage every aspect of their patients’ care, especially those with chronic diseases, partnerships like this reaffirm the crucial role they play in the current contracting landscape,” said Michael Petrizzi, MD, Hanover Family Physicians and member of VCP’s Payer Committee. “Now physicians have support in meeting the goals CMS established by appropriately compensating physician time and effort to deliver comprehensive, high quality care.”

The Transcend team will work directly with VCP physician office staff to assist with patient data review and analysis, and identify gaps in care. Transcend’s care navigation nurses, for example, will work side-by-side with VCP staff to review patient charts and contact patients who are overdue for appointments or would benefit from chronic disease management programs such as diabetes management.
