Upcoming webinars address the importance of care team collaboration and targeting high-risk patients to improve care transitions

The complimentary webinars cover important topics, including streamlining care team communications and improving care transitions for high-risk patients. The webinars are designed to provide clinicians and other healthcare professionals with advice and tools they can use within their own healthcare organizations.

On Thursday, January 13, 2016 from 1:00 – 2:00 p.m. ET, the series will start with an interactive webinar titled “How Advocate Medical Group optimized care team communication.” During this session, Advocate Medical Group’s Director Jennifer DeBruler, MD and PerfectServe CEO Terry Edwards will discuss best practices and lessons learned from Advocate Medical Group’s development of a new contact center that streamlined communications and connected its care teams more efficiently by effectively managing the complexity of call schedules and physician preferences. On February 16, 2016 from 1:00 – 2:00 p.m. ET, Eric Coleman, MD, MPH, Professor of Medicine and head of the division of Health Care Policy and Research at the University of Colorado and the director of the Care Transitions Program at the school’s Anschutz Medical Campus, will host a webinar titled “Risk targeting for complex care transitions.” The webinar will focus on identifying high-risk patients within a population and delve into the mechanics of predictive metrics for risk targeting. He will also discuss how to gain perspective on the use of metrics for improvements in coordinated care, as well as how to manage expectations with the use of risk tools for targeting patients. Finally he will provide insight on how engagements with other community providers can impact care transitions for high-risk populations.
