On February 16, 2016 from 1:00 – 2:00 p.m. ET, Eric Coleman, MD, MPH, Professor of Medicine and head of the division of Health Care Policy and Research at the University of Colorado and the director of the Care Transitions Program at the school’s Anschutz Medical Campus, will host a webinar titled “Risk targeting for complex care transitions.” The webinar will focus on identifying high-risk patients within a population and delve into the mechanics of predictive metrics for risk targeting.
