Miami Beach Community Health Center Obtains Highest Level Designation as a ‘Patient Centered Medical Home’ from NCQA

MIAMI--()--Miami Beach Community Health Center (MBCHC) obtained recertification for its three locations as a Level 3 Patient Centered Medical Home by the National Committee for Quality Assurance (NCQA). Level 3 is the highest designation given by the NCQA. “Our primary care physicians work collaboratively to provide comprehensive care and disease management, taking into consideration a patient’s linguistic, cultural, and educational needs,” said Trudy Edwards, Director of Grant and Project Development.

The patient-centered medical home is a model of care that emphasizes coordination and communication to transform primary care into ‘what patients want it to be.’ Patients in medical homes receive high quality care through a strong patient-provider relationship. The NCQA Patient Centered Medical Home Recognition is the first and most widely used formal assessment program based on six standards:

  • Patient-Centered Access
  • Team-Based Care
  • Population Health Management
  • Care Management and Support
  • Care Coordination and Care Transitions
  • Performance Measurement and Quality Improvement.

MBCHC embraces this model of primary care and has met the standards set by NCQA in all of its locations. The medical home team works to provide a superb and personalized experience to patients, while incorporating a systems-based approach to quality and safety.
