OWINGS MILLS, Md.--(BUSINESS WIRE)--Integra ServiceConnect®, a community-based healthcare support company, recently introduced its unique service program in Illinois for a Medicaid health plan serving high-need individuals in Cook County. The program will complement the plan’s existing interdisciplinary care team and clinical programs, extending the reach of these programs into the community to support the plan’s most underserved members. The program initially will serve Medicare-Medicaid Alignment Initiative (MMAI) members in Cook County.
Integra employs community health workers called Community Coordinators to assist difficult-to-reach members who have physical and/or behavioral health needs requiring high-touch, personalized outreach, support and service coordination. These individuals are often homeless, transient, or shut-in, encountering significant social, behavioral and environmental barriers in accessing health care and social services that can improve their health status. Integra helps these members access medical services needed to close important gaps in their health care by targeting the barriers that prevent them from doing this on their own.
The company specializes in working with high-need, underserved individuals whom health plans and providers are unable to reach through traditional outreach efforts such as phone calls and mailings. Community Coordinators are hired locally – from and around members’ own communities. They are non-clinical staff, specially trained to work face-to-face in the community with these members to address the social, behavioral and environmental factors known to account for 80% of health outcomes.
