CareEvolution Introduces Apple Watch App to Connect Trinity Health Patients and Providers to Each Other

Apple Watch App Expands Ability to Deliver Clinical and Operational Alerts and Track Patient Health

ANN ARBOR, Mich.--()--CareEvolution, Inc. announced today the cFHR™ Apple Watch App that connects patients with hospital-based care givers and providers across the care continuum for Trinity Health, a leading national health system with 85 hospitals and 128 continuing care facilities in 21 states.

The new cFHR™ Watch App from CareEvolution is the latest in a series of innovations to help Trinity Health connect with the people they serve, making it quicker and simpler for patients to receive alerts, review new medical information and participate in the management of their health right from their wrist. The app is available to Trinity Health patients who already are using the cFHR™ HealthKit enabled iPhone app that CareEvolution launched in December 2014.

This initiative builds on an existing effort with Trinity Health to implement CareEvolution’s HIEBus™ health information exchange infrastructure across its enterprise. “We are connecting the existing digital islands of automation to create a comprehensive patient record that integrates data from within the Trinity Health enterprise with data from payers and community-based providers. This strategic data infrastructure today powers our transformational apps across Trinity Health such as care coordination, analytics and clinical surveillance, as well as traditional provider and patient portals,” said Vik Kheterpal, MD, a principal at CareEvolution.

The cFHR™ Apple Watch App will be available on the Apple Watch App store beginning April 24. The cFHR™ app for iPhone is available now as a free download from the App Store.

CareEvolution will be previewing the new app this week at the Interoperability Showcase at the HIMSS 2015 conference in Chicago, Illinois.
