Cape Cod’s Emerald Physicians ACO Selects eClinicalWorks for Population Health Management

Medicare Accountable Care Organization Chooses eClinicalWorks Care Coordination Medical Record Solution to Advance Program Goals

WESTBOROUGH, Mass.--()--eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that Emerald Physicians ACO of Hyannis, Mass., has selected eClinicalWorks Care Coordination Medical Record® (CCMR) for advancing its physician-led Accountable Care Organization (ACO) objectives, including coordinating care across the continuum, evaluating population health and quality improvement activities.

“Emerald Physicians ACO delivers coordinated, high-quality care for its patients at an improved cost,” said Dr. Cormac Coyle, medical director of Emerald Physicians ACO. “As a primary care-centric ACO, we turned to eClinicalWorks for population health management because of its strong track record in this arena. Working with eClinicalWorks will provide the level of insight into our patient populations and care coordination needed to reach our goals.”

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship. Emerald Physicians ACO participates in the Medicare Shared Savings Program, which has been created by Centers for Medicare & Medicaid Services (CMS) for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries, improve patient satisfaction and reduce unnecessary costs. eClinicalWorks CCMR will aid Emerald Physicians ACO in meeting its quality measures and performance metrics.

“Emerald Physicians ACO is making great strides in providing holistic, quality care,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks delivers the top-rated population health management solution in the market, a comprehensive platform with powerful analytics, care coordination, patient engagement and a referral network. We look forward to working with Emerald Physicians ACO to help them meet the program goals.”

eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including CMS, to provide cost and utilization for services delivered to patients.

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
