• Adults/Youth: The Short-Form Behavioral Health Assessment (SF-BH ^TM) and the Consumer Health Inventory (CHI) integrate an individual’s behavioral health services with his or her physical health services by including questions related to the individual’s recent level of physical health functioning and strengths. Individuals can print and share an electronic health status report with their provider instantly and keep a copy for their personal records.
• Children: The Consumer Health Inventory – Child Version (CHI-C) is completed by caregivers and supports children and their parents/guardians in identifying a child’s strengths, level of behavioral health need and number of school absences, all of which have been identified as being important indicators of overall effective behavioral health functioning.
